Title: Registered Nurse Supervisor: Director of Nursing Qualifications: · Graduate of an accredited nursing program. · Has current, valid license to practice as a registered nurse in Colorado. · Compassion and empathy for people with end-of-life issues. · Minimum of two years hospice experience or comparable field preferred. · Strong commitment to the value that “Palliation is a valid and appropriate choice for people at any stage of their encounter with potentially terminal disease.” · Strong commitment to the Namaste Hospice Vision Statement. · Ability to adjust to changing conditions. · Ability to establish a caring and empathetic relationship with patients. · Demonstrate ability to develop, follow the Plan of Care under the direction of the Medical Director. · Evidence of maturity and ability to deal effectively with job demands · Demonstrate verbal and written communication skills. · Demonstrate familiarity with state and federal hospice regulations. · Owns reliable transportation, has a Colorado Driver’s License and auto liability insurance. · Able to maintain high level of integrity, accountability and confidentiality in one’s work as a member of an interdisciplinary team. · Able to employ self-initiative, planning and organizational skills in working with a high volume yet ever-changing patient caseload. · Able to serve responsibly and compassionately in a context of differing developmental and situational needs including age, race and socio-economic backgrounds. · Able to perform physical demands of the job including walking, lifting, pulling, driving, step climbing and travel. · Possesses knowledge of pain and symptom control as related to terminally ill patients and pursues ongoing education in this area; dedicated to pursuing national certification in hospice within two (2) years of employment. General Responsibilities · Assumes oversight for assessment, planning, implementation, documentation and evaluation phases of the nursing process. · Performs C.N.A/P.C.P supervisory visits every two weeks. Delegates tasks as appropriate. · Contributes to the Interdisciplinary Plan of Care seeking input from other team members regarding the patient plan of care to obtain additional knowledge and support. · Facilitates re-certification process as required by state and federal regulations. · Initiates communication with other hospice staff members and other agencies as needed to coordinate optimal care for the patient/family. · Completes visit notes, daily logs and payroll in a timely manner in compliance with NCC procedures. · Writes concise, easily read visit notes. · Complies with Medical Records Manager CQI request within 24 hours. · Maintains regular communication with attending physician. · Responsible for obtaining all physician orders, including medication orders. · Orders pharmaceutical medications, durable medical equipment and supplies in a timely manner; ensures delivery and pick-up; identifies and documents appropriateness for hospice coverage. · Assesses and documents changes in level of hospice care with another member of the IDT. Communicates changes to Medical Records and the IDT. · Manages time effectively and shows dependability for getting things done. · Maintains satisfactory work attendance. · Wears neat and professional attire including name badge during work hours. · Performs duties in compliance with NH Policy and Procedures. · Performs other duties as assigned. Professional Development · Attends staff support meetings and other meetings as required. · Plans and prioritizes goals. · Seeks to improve professional skills by attending seminars and other training opportunities. · Attends all DT meetings having less than three unexcused absences in one year. · Participates in 12 NH ongoing in-service education programs each year. Teamwork · Creates positive image between co-workers · Promotes positive and consistent intra-agency communication. · Role models team work, accountability, and a positive attitude. · Shows commitment in the value “Respect for one another” by demonstrating appreciation of diversity and difference internally toward team members. Assumes positive intent. · Communicates effectively and clearly ideas and information. Listens well and maintains confidentiality. · Participates constructively in team problem solving process. · Has a commitment to support the success of the team. · Adheres to the value of creating a true interdisciplinary team assuring equal and active roles for all members. Special Activities · Shares on call duties. · Thinks strategically about the future. Embraces opportunities. · Participates in agency and community programs as requested to further the vision of Namaste Comfort Care to “Shift the dominant paradigm regarding the way the world view serious and life limiting illness, the process of decline and death itself.” · Facilitates positive relations with other medical professionals in the community. Delivery of Care · Provides patient updates via voicemail for on-call purposes. · Coordinates appropriate ancillary therapies for patients (PT, OT, ST). · Provides appropriate support at the time of death. · Instructs primary caregivers, volunteers and employed caregivers to provide care as indicated including general information to nursing facility staff concerning hospice services, pain management and end-of-life care. Documents all instructions. · Reports any changes in patient condition or family situation by notifying the rest of the team. · Follows the Interdisciplinary Plan of Care for each patient |
